Please fill out the application form below clearly, comprehensibly and completely, and submit it to us so that your request addressed to MOLECIA KİMYA ANONİM ŞİRKETİ under the Law No. 6698 on the Protection of Personal Data may be fulfilled.

Your application submitted to us will be responded to as soon as possible and no later than thirty days, depending on the nature of the request. If the information and documents submitted in the application are incomplete, insufficient or not suitable for understanding, we may contact you in order to clarify the application.

Our Company reserves the right to request additional information and documents for identity and authorization verification in order to prevent legal risks that may arise from unlawful and unfair data sharing, and especially to ensure the security of your personal data. If the information submitted within the scope of the form is not accurate or up to date, or if an unauthorized application is made, our Company shall not be responsible for any claims that may arise due to incorrect information or unauthorized application. All responsibility arising from applications that are unlawful, contrary to legislation, misleading or incorrect shall belong to the applicant.

Within the scope of the Communiqué on the Procedures and Principles of Application to the Data Controller, if the response to be provided exceeds ten pages, a processing fee stipulated under the relevant legislation may be requested for each page after the tenth page.
